Login to your account and click on the “My Submissions” tab. Scroll down until you see the “Service Request ▼” drop down menu.
 
SERVICE REQUESTS CHOICES:

  • General Change: Update your address, replace warn/damaged credential.
  • Lost/Stolen: Report your credential lost or stolen and order a new one.
  • Renewal: You're still actively serving, and your credential is expiring or expired.
  • Category Change – 926B to 926C: Transitioned out of Law Enforcement and served 10 plus years or discharged from Law Enforcement due to a medical condition and served less than 10 years. (Note: A disability rating from the VA does not waive the 10-year requirement.)
  • Category Change – 926C to 926B: Returned to Law Enforcement.

PURPOSE: To determine eligibility of requesters and process applications for DON law-enforcement credentials. The applicant's Social Security Number is solicited solely for the purpose of validating identity.

ROUTINE USE(S): Information will be disclosed to law enforcement authorities and administrative support staff assigned to the issuing agency with a need to know in order to verify Title 18, Section 926B and 926C eligibility requirements and process applications. Additionally, the answers and any information resulting therefrom may be disclosed for use in a criminal or administrative proceeding if the applicant knowingly and willfully provided false statements or information.

DISCLOSURE: Voluntary. However, applicants that fail to provide the requested information will be denied issuance of DON law-enforcement credentials.

The public reporting burden for this collection of information, OMB 0703-0067,(AAKM), “Law Enforcement Officer Safety Act (LEOSA) Credentialing Program, is estimated to average 30 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. Send comments regarding the burden estimate or burden reduction suggestions to the Department of Defense, Washington Headquarters Services, at whs.mc-alex.esd.mbx.dd-dod-information-collections@mail.mil.
